Anaphylaxis Management

  • Epinephrine 1:1000 0.5ml IM every 5 minutes (.01ml/kg)
  • Respiratory support: 
  1. Oxygen 6L/mt by FM or NC
  2. Intubate if needed
  3. Albuterol 0.5% 0.5ml in 2.5ml NS q 30 minutes by Nebulizer
  • H1 blocker: Diphenhydramine (Benadryl) 1mg/kg IV or IM (max 50 mg) DO NOT GIVE BEFORE EPINEPHRINE
  • H2 Blocker: Ranitidine 1mg/kg IV
  • Steroids: Solumedrol 250mg IV once followed by 125 mg IV q6h
  • Inotropic support:
  1. Dopamine 2-20 microgram/kg/minute IV, titrate to obtain SBP >100
  2. OR
  3. Levophed (norepinephrine 8-12 micrograms/minute IV , titrate to obtain SBP >100
