Acute stroke with Atrial Fibrillation

Explore effective management strategies for acute stroke in Atrial Fibrillation among non-TPA candidates.

Acute Stroke in Atrial Fibrillation , non-TPA candidates:

  • May start patient on Aspirin instead of Heparin
  • IV Heparin is a consideration , but avoid if the stroke is large
  • SQ Heparin for DVT prophylaxis if significant leg weakness present


  • Initiate Warfarin 3 days after the event, after a repeat CT scan
  • Aim to make INR therapeutic by 7th-10th day
  • Delay warfarin if significant hemorrhage is present
